MSMC Patient Online Services- Online LOA Request



I. In order for us to serve you better, LOA must be filed 3 days prior to your availment date. Please allow us to process your request at least 24hrs once submitted.

II. Requests beyond office hours (07:00AM-05:00PM Except Holidays) will be processed on the next business day.

III. For approved request, our HMO will forward the LOA to your specified e-mail address and must be printed and presented on the day of your check-up/appointment.

IV. If no feedback is given by our HMO within 24 to 48 hours, you may contact us at (046) 230-9637 | (02) 8584-4594 to 96 for assistance.
